Who Qualifies for Hospice Care?

You May Qualify If:

  • You are a patient who has a terminal illness
  • Your medical prognosis is deemed to be six months or less
  • You may continue to qualify for services beyond six months if your doctor and healthcare team agrees that the program is the best option for your ongoing care.
